Dave Koz & Friends At Sea Cruise Cabin Giveaway Contest.
NO PURCHASE NECESSARY. All entries shall become the property of www.davekozcruise.com and may be used for future email blasts regarding Dave Koz. Not responsible for lost, misdirected, delayed or incomplete entries.
CONTEST RULES:
1. The contest is open to all attendees at least 21 years of age or older.
2. The prize is not redeemable in cash and must be accepted as awarded.
3. Decisions of the contest judges are final - no substitutions will be available.
4. By claiming the prize, the winner authorizes the use, without additional compensation of his or her name and/or likeness for any and all advertising or promotional purposes (including, but not limited to, radio broadcasts, newsletters, or on www.davekoz.com or www.davekozcruise.com).
5. In accepting the prize, the winner and guest acknowledges that www.davekoz.com, Visible Image, LLC, Jazz Cruises, LLC, W. F. Leopold Management, Inc., Just Koz Entertainment, Inc., Bridgeway Entertainment, Inc., Agency for the Performing Arts, Jazz Cruises, LLC and any respective affiliates, employees agents and/or representatives may not be held liable for any loss, damages or injury associated with accepting or using this prize.
6. Winners and guest must sign a release form as prepared and accepted by www.davekoz.com acknowledging their acceptance and understanding of the complete contest rules.
7. This contest is subject to all federal, provincial and municipal laws.
8. If prize involves travel to port, winner and travel companion must agree to the airline's Terms and Conditions of travel. Once confirmed, trip dates and travel accommodations cannot be changed by winner; however, travel is subject to availability, blackout dates, and/or change due to circumstances beyond Sponsors' and Administrators' control. Winner and travel companion must travel on the same dates, itinerary, and fare class. No extensions will be permitted, and any unused travel will be forfeited.
9. One entry per person.

Winner will be determined by random drawing and will be notified by e-mail and/or telephone notification. A single prize of a cruise will be awarded on September 4, 2008.

Please complete the form to the right in order to be eligible to win a free Deluxe cabin with Veranda for two on the Dave Koz & Friends at Sea Cruise, which is sailing from Ft. Lauderdale from November 2-9, 2008. Dave’s special guests include Eric Benet, RnR (Rick Braun & Richard Elliot), Jonathan Butler, George Duke, Candy Dulfer, Jeff Golub, Najee, Brian Simpson, Deniece Williams and Special Guest: Tower of Power and many others. The "free cruise" includes the cabin, all meals, ports-of-call and all on-board shows and entertainment. Air and ground transportation, alcohal, excursions, purchases, port charges, taxes and gratuities are not included in the prize.
